Thank you for booking your appointment at Bounce Physio

If you booked an Assessment Consult:

  • Please arrive at least 10mins before your scheduled appointment time. If you have been unable to complete your form online prior to your appointment, please arrive 15mins early.
  • Bring along any relevant medical information including:
    • Referrals from your GP, specialist or other health professionals
    • Radiology Reports - X-Ray, CT/MRI Scans etc
      NOTE: You do not require a referral to make an appointment at Bounce Physio
  • If possible, wear comfortable, loose-fitting clothing that you can move in and that will allow us to easily see the affected area (where appropriate).

Paying for your appointment

We have accept payment from a range of payer types. Please consult the list below BEFORE your appointment to make sure that you are prepared for payment with the appropriate cards and/or documents.

Private / Health Funds

What we need:

  • Your private health fund card (actual card, can't accept card no./electronic card)
  • All physios are HCF More For Muscles Providers and our Hendra physios are Bupa Members First Providers

Is someone else paying for your appointment?

If your appointment is being funded by another payer (eg. your employer, WorkCover, insurer, Medicare, DVA, NDIS etc) please see below for details about your payment type.

PLEASE NOTE: 
You will need to pay for your consult in full at the appointment if you do not have:

  • Required documents
  • Full approval status for a claim

Medicare Plans (CDM/EPC)

What we need:

  • Your eligible Medicare plan (CDM / EPC) from your GP

If you require bulk-billing - Call our reception team to check that your physio offers this, as private fees apply to some physios

DVA

What we need:

  • A copy of a current DVA referral from your GP
  • DVA document showing your DVA number, Card Type (ie. White/Gold/Gold TPI), Approved Conditions

WorkCover / Employer Self Insured

What we need:

  • Your Claim No. and case manager details - call us after booking online
  • Your current WorkCover Medical Certificate from your GP -a valid, current copy must be provided to us at all times throughout the claim

Full payment at the time of appointment is required until your claim is approved.

NDIS

What we need:

  • Details of your NDIS plan management type, plan manager contact details - call us after booking online
  • Your NDIS plan / goals

Services available for Self-Managed and Plan-Managed / Agency-Managed NDIS clients only, we are not eligible to provide services to NDIA-Managed NDIS clients

Third Party Insurance Claims

What we need:

  • Your Claim No. and case manager details - call us after booking online
  • Any documents related to your claim status/conditions eligible for treatment

Full payment at the time of appointment is required until your claim is approved.
